Yeah, you're definitely doing it wrong, because GR does not "invent" lone MIDI files. Also that BMP you've created a hexdump of, must be shown up on "Sprites" tab at least.

But okay, no pressure, let's just use the files you've extracted with Game Extractor.

Like I said, GR doesn't give a shit about extensions, it scans all files for known magic bytes and that's it. See the source yourself.

Hmmm, not sure. If the game was released in 1996 then there's a chance that it was using 8-bit PCM samples, and that's the reason for the low quality. Although it is possible that you have some conversion issues, because Sound Blaster 16 (the first sound card that could handle 16-bit PCM under DOS) was already released by that time if my memory serves, so there's a chance of 16-bit PCM samples as well (with 44100 sampling rate that would be CD quality).

You should detect the correct sample size in those audio files first. Could you attach an example audio file? Maybe I'll be able to tell more if I can analyze it.
